Information provided on this form, and throughout the application process, is necessary for the Office of Continuous Professional Development for processing your application, compiling general statistical information, and administering any and all programs and processes under the auspices of the Office of Continuous Professional Development. IPG Programs: The application fee of $250 (plus HST) is non-refundable and is valid for two years from the date of application. Students who withdraw during the first week of the CPS session will receive 100% refund of the tuition fee. A 75% refund is possible until the end of week two of the session, and a 50% refund can be processed until the end of the third week of a session. After the third week there will be no refunds. The same refund policies apply to both CPS 1 and CPS 2.
CPD Programs: No refunds are available for these programs. Registration fees are for the educational program and meals as specified on the agenda only (where applicable). All other costs (for example: travel, accommodation, other meals, etc.) are the responsibility of the participant. In the event of program cancellation, refunds are limited to program registration fees only. All program prices are subject to change, and may be changed at any time.
The University of Toronto, Faculty of Pharmacy reserves the right to cancel or modify events/programs at any time. Registrants will be notified at the earliest possible date in the event of a cancellation. Tuition fees for programs cancelled by the University will be refunded; however the University will not be liable for any loss, damages or other expenses that such cancellations may cause. Participants must complete all requirements in order to receive a Continuing Professional Development certificate.
